CMS moves to make Medicare drug-price negotiation permanent
On June 16, CMS issued a proposed rule (CMS-4215-P) that would convert the Medicare Drug Price Negotiation Program from a guidance-based experiment into permanent regulation. It codifies every phase of the cycle — drug selection and ranking, manufacturer agreements, the offer-and-counteroffer process, renegotiation, and publication of maximum fair prices — starting with initial price-applicability year 2029. Comments are open through August 17, with a final rule expected in fall.
Why it matters to you: this is the machinery behind the prices your elderly inpatients actually pay at discharge. A permanent framework means the negotiated-price list and Part D formulary shifts become a stable planning input rather than a year-to-year guessing game — relevant when you're choosing between a negotiated agent and a pricier alternative on a fixed-income patient.
Nothing to act on tonight — this is a 2029 rule in a comment window. File it as the reason your hospital's formulary/pharmacy committee will be busier this fall.

Update: Iran deal holding — ships through Hormuz, signing set for Friday
The agreement you watched land this week is sticking. Ships are now confirmed moving through the Strait of Hormuz along the southern route through Omani waters, the U.S. naval blockade is being lifted, and a formal signing is slated for Friday, June 19. The big caveat is logistical: officials warn mine-sweeping could run 40–50 days before insurers and shippers fully trust the waterway, so the oil that backed up in the Gulf won't all flow at once. Iran's nuclear program stays parked in a 60-day negotiation window.
Why it matters to you: same thread as yesterday — crude eased and that should slowly bleed pressure out of the supply-and-logistics inflation that's been squeezing hospital budgets. "Slowly" is the operative word given the minesweeping timeline; don't expect contracted supply costs to drop overnight.
Talking point: the war's actual trigger — the nuclear program — still isn't settled. Friday's signing ends the shooting, not the argument.
