What happened: STAT reported today that hospital leaders who welcomed the rural health fund — the money Congress added to soften the Medicaid cuts — found in the fine print that only 15% can go to direct patient care, and that systems in at least 25 states have to shrink services to qualify, including dialysis, labor and delivery, or converting to a designation that eliminates inpatient beds altogether.
Why it matters to you: When the small hospitals around Plant City stop admitting, those patients don't disappear — they arrive on your service at 2 AM, sicker and farther from home, and the nursing home or dialysis chair you were counting on for the discharge may not exist anymore.
Do this: Next time a transfer comes in from a small facility, ask case management directly what that hospital still does and what it has stopped doing. The answer has been changing quietly.

What happened: Comments on Medicare's 2027 physician payment rule were due today, September 14. That's the rule carrying the government's request for input on whether the American Medical Association's ownership of the billing-code set does harm, and whether there should be a free alternative — the fight you read about here Friday.
Why it matters to you: Nothing changes at the bedside tonight. But the comment file is now closed and public, and whatever lands in next year's rule was decided by who wrote in during the last six weeks.

Hospital doctors went on strike today for the first time in American history. Was walking out the right lever?
Why it's live: About 150 physicians represented by Doctors Council SEIU began a four-day unfair-labor-practice strike this morning at Allina Health's Mercy Hospital in Coon Rapids and its Unity campus in Fridley, Minnesota — believed to be the first walkout by private-sector hospital doctors in U.S. history. They unionized in 2023, have met the system more than 60 times without a first contract, and authorized the strike with 90% support in July. Hospitalists are at the center of it. Hanging over everything is a proposed merger with Sutter Health that would create a $26 billion, 39-hospital system, now under review by Minnesota's attorney general.
Three years of bargaining produced no contract, so the only leverage left is the one every other worker has. The core asks aren't exotic: dedicated paid sick days (Minnesota law gives every worker six; Allina's physicians say theirs come out of the same pool as vacation), cost-of-living raises after years of inflation, an end to non-competes, and formal physician input on clinical decisions and on how AI gets deployed. The argument is that workload set by people who don't carry a panel is a patient-safety issue, not a pay issue.

Allina says the union speaks for roughly 10% of the physicians credentialed to work at those two hospitals, that both will run normally with no interruption to services, and that half the bargaining unit didn't walk — its spokesman Blois Olson put it at 37 who crossed the picket line and 40 not scheduled this week. The broader argument is that a hospital's wage bill can't be set in isolation from what payers actually pay, and that the table, not the sidewalk, is where a first contract gets written. There's also the uncomfortable part nobody on either side enjoys saying out loud: a strike only works as leverage to the degree it threatens care.
"We will continue to work with both unions to reach fair agreements, but these negotiations cannot be divorced from the economics that are impacting both patients and health care systems." — Allina Health statement.
Where it lands: The measurable question is whether four days moves a table that sixty sessions didn't. The bigger question underneath is whether employed physicians — now the large majority of American doctors, hospitalists most of all — are professionals who negotiate individually or employees who bargain collectively, and whether consolidation on the scale of an Allina-Sutter merger leaves any third answer. More than 60 Allina hospice nurses have been out since Friday on a separate seven-day strike.

The Oman-hosted talks on temporary Hormuz shipping routes — Iran, Iraq and the Gulf states, due to meet in Salalah today — were called off Sunday night with no new date, Iran citing "events unfolding in Yemen." Those events: the Houthis say they've taken the Greater and Lesser Hanish islands in the southern Red Sea, and that Saudi jets flew 54 strikes in 24 hours. Puerto Rico. Two island-scale outages in one day. Around midday a unit at Central San Juan tripped and took out more than 170,000 LUMA customers; in the evening a unit at Central Aguirre tripped and took out another 122,269, San Juan hardest hit both times.
