Good morning, Marco. Coffee first — this one's worth five minutes before you sleep. Two things genuinely matter to your next set of admissions: the Cyclospora numbers out of Michigan are an order of magnitude bigger than the federal count suggests, and there's fresh CDC data on an antibiotic-resistance gene that quietly breaks most of what you'd reach for empirically. Everything else is lighter — the Mets started dismantling the roster overnight, Cuba's grid fell over again, and Trump says he's talking to Tehran today instead of bombing it.

Cyclospora: Michigan alone now reports 9,680 cases — the federal count is badly lagging reality

The national picture you've been tracking (6,707 lab-confirmed cases across 45 states, 423 hospitalizations, plus roughly 11,500 illnesses still unconfirmed) is the floor. State health departments are further ahead: Michigan's MDHHS now reports 9,680 cases with 160 hospitalizations on its own, and northwestern Ohio reports 2,785 more, with 1,173 in Lucas County alone. For scale, the entire United States recorded about 4,500 cases in 2023 and 3,500 in 2024.

Michael Osterholm and Craig Hedberg (University of Minnesota) both told CIDRAP the iceberg-lettuce arm of this — Taylor Farms de México, recalled July 17 — is probably in its waning phase; the epi curve shows two peaks and we're on the back side of the second. But the second, unrelated outbreak the FDA flagged has grown from 72 to 93 cases with the source still unknown, and Hedberg points to a large unexplained cluster in North Carolina and the southeastern states that may be it. Why it matters to youOsterholm's timeline is the operational point: 7–10 day incubation, then people don't seek testing until they suspect a specific parasite, then another 7–10 days for results. Two and a half to three weeks minimum from exposure to a confirmed case. The elderly patient who presents to you tonight with three weeks of watery diarrhea, cramping, anorexia and 8 pounds of weight loss was exposed in mid-July, and nothing in the federal count reflects them yet. Florida isn't in the nine-state lettuce cluster — but the unexplained second outbreak is clustering in the Southeast.

Suggested actionProlonged non-bloody diarrhea in an older adult this month should get a multiplex GI PCR up front, not a stool culture and O&P. Confirm your panel actually includes Cyclospora cayetanensis — not all do. TMP-SMX remains first-line; have the sulfa-allergy alternative pre-decided rather than paged about at 3 a.m.

NDM-carrying CRE up 461% in four years — and most labs can't tell you it's NDM

CDC data highlighted by CIDRAP show infections from carbapenem-resistant Enterobacterales carrying the New Delhi metallo-β-lactamase gene rose 461% between 2019 and 2023. Among carbapenemase-producing isolates, the blaNDM share went from 5.4% in 2016 to 39.8% in 2023, and NDM now accounts for 73% of carbapenemase-producing E. coli. CRE mortality runs 30–50% and the organisms preferentially find patients who are already sick and already in the hospital.

The clinical wrinkle: the newer β-lactam/β-lactamase-inhibitor combinations built to defeat KPC do not cover NDM. Aztreonam-avibactam, FDA-approved in early 2025, does — but only helps if someone establishes the infection is NDM-mediated, and many clinical microbiology labs lack the molecular testing to make that call. Why it matters to youYou admit the exact denominator: elderly, recently hospitalized, post-acute-care residents, indwelling devices, prior carbapenem exposure. On nights, the empiric decision is yours and the ID consult is hours away. If your reflex for a sick urosepsis admission with prior CRE history is meropenem or ceftazidime-avibactam, the resistance mechanism now matters more than the species name — and you may not learn which one you're facing for days.

Suggested actionOne question for your micro lab this week: can we do carbapenemase gene typing in-house, and what's the turnaround? If the answer is "send-out, five days," that's worth raising at your next group meeting — it's the difference between a pre-decided aztreonam-avibactam pathway and improvising.

Iran: Trump calls off a planned strike, says negotiations with Tehran begin today

After ten days of alternating barrages and short pauses, Trump said late Sunday that he had cancelled a planned attack because a deal is near, and that negotiations with Tehran would begin Monday. Al Jazeera's live coverage has moved to a new thread headlined on Iran urging Washington to honour the June 17 memorandum of understanding — the 14-point framework covering Hormuz navigation, the nuclear and missile programmes, and sanctions relief. Why it matters to youPractically: this is the variable behind the crude price you've watched swing from $100+ to the low $80s and back. Talks starting is the first real de-escalation signal since the late-July strike cycle — but "talks begin Monday" has preceded a resumption twice already this month, so hold it loosely.

Should Congress scrap Medicare Advantage quality bonus payments?

Why it's live right now: A federal judge in Georgia threw out 20 of the measures CMS used to calculate Clover Health's 2026 star rating, worth roughly $120 million to the company. CMS responded by "voluntarily recalculating" star ratings for every MA plan — but only upward, never downward — and Elevance, SCAN and Alignment have all since filed suit seeking the same treatment. Bonus spending hits $13.4 billion in 2026 by KFF's count, or about $16 billion by MedPAC's.

The case FOR scrapping it

A quality program that only ever pays out more, never less, has stopped measuring quality and started functioning as a rate increase with extra paperwork. That's the argument from Erica Socker and Katie Keith, writing in Health Affairs Forefront: the recalculation is a one-way ratchet in which plans that changed nothing get paid more, while the measures CMS stripped are precisely the ones plans scored worst on — appeals review, members choosing to leave, SNP care management. Peer-reviewed work has found the program neither improved plan quality nor helped beneficiaries compare plans meaningfully; ratings are still calculated at the contract level, where a single contract can span 271 plans across 49 states. MedPAC has recommended replacing it outright with a budget-neutral value incentive program, and CBO scored elimination at nearly $100 billion over ten years back in 2018 — higher now.

The case AGAINST scrapping it

Strip the bonuses and you don't get $13 billion of savings — you get $13 billion of benefits taken away from seniors who are already being squeezed. Bonus dollars flow into rebates, which is what funds dental, vision, hearing, transportation, OTC allowances and $0 premiums; supplemental benefits were already thinning for the 2026 plan year. Better Medicare Alliance, led by Mary Beth Donahue, argues star ratings are also the main mechanism aligning plans and providers around shared outcome goals, so "any major changes to the program would impact not only health plans, but also providers." AHIP's Chris Bond frames the timing as the real issue — plans are absorbing new star ratings policy "during this time of sharply rising medical costs." Fix the measures, this side says; don't demolish the only payment lever tied to clinical performance at all.
