What happened: The Pennsylvania health department's statewide update, posted Friday afternoon, put the outbreak at 767 confirmed cases in 38 counties, up from 693 a week earlier; the state held 11 pop-up clinics in two days and gave more than 400 MMR doses.
Why it matters to you: This is the state-level number last night's report said would become the only real picture, now that the federal dashboard counts deaths differently — and it is still climbing by roughly seventy cases a week.
Do this: Keep treating fever-plus-rash in any adult as airborne until proven otherwise — mask and isolate at the door, before the history.
What happened: The CDC's Friday respiratory update says the amount of illness sending people to care is very low, flu and RSV are both low, but COVID activity is elevated and rising nationally, and rhinovirus/enterovirus and group A strep are increasing.
Why it matters to you: For the next few weeks an older patient arriving overnight short of breath is more likely COVID or a bacterial pneumonia than influenza, so empiric oseltamivir is mostly wasted and a strep-positive pharyngitis in an adult is worth taking seriously rather than filing as viral.
Do this: Keep COVID on the front of the respiratory panel for admissions this month, and don't let a low flu number talk you out of testing when the story fits.
Why it's live: On Friday the HHS inspector general released two audits finding that HumanaChoice and UnitedHealthcare of Wisconsin submitted diagnosis codes their own medical records didn't support — roughly $180 million in extra payments across 2020 and 2021 once the sample was extrapolated to the full contracts. Both insurers called the methodology flawed and said they won't return the money. Two days earlier, House Ways and Means Republicans advanced the Apples to Apples Comparison Act on a party-line vote, which would change how the Medicare Payment Advisory Commission studies Medicare Advantage spending. MedPAC currently estimates Medicare pays 14% more per Medicare Advantage enrollee than it would in traditional Medicare — about $76 billion a year.
Rep. Aaron Bean, one of the bill's sponsors: "When the Medicare Payment Advisory Commission (MedPAC) compares apples to oranges, policymakers are left in the dark."
The case: MedPAC's headline number stacks a managed population against unmanaged fee-for-service without adjusting for who enrolls in which, so the "overpayment" partly measures a difference in populations, not in billing. A coalition of 22 groups organized through AHIP, the insurers' trade association, argues the comparison should be standardized before Congress legislates off it. On the audits specifically, a federal court already vacated the rule that let CMS extrapolate from a sample to a whole contract, in a case Humana brought — so reading $180 million out of 250 sampled enrollee-years is exactly the method a judge threw out.
Paul Ginsburg of the USC Schaeffer Center, in comments filed on the bill, warns it would undermine MedPAC's ability to give Congress independent advice by legislating analytic results more favorable to insurers.
The case: MedPAC is nonpartisan and reports to Congress precisely so that the payment math isn't written by the people being paid. If the methodology is wrong, the remedy is a better method argued in public, not a statutory instruction to produce a friendlier figure. And the audits aren't a modeling dispute — for HumanaChoice, records failed to support 178 of 250 sampled enrollee-years; for UnitedHealthcare of Wisconsin, 183 of 250. Sampling is how every other Medicare audit works.
Where it lands: The bill cleared committee, not the House, and CMS's appeal of the extrapolation ruling is still pending — so neither the number nor the clawback authority is settled. For a hospitalist the downstream question is the one that reaches the floor: risk-adjustment pressure is why documentation queries keep arriving about diagnoses you didn't treat.
